Good-faith statement

Accuracy statement
A statement under penalty of perjury that the information is accurate and you are authorized to act.
If material was removed by mistake.
If you believe content was removed or disabled because of mistake or misidentification, you may submit a counter-notification. Include your contact information, identification of the removed material, a good-faith statement under penalty of perjury, and consent to appropriate jurisdiction where required.
